How do I get my License Back



 
  • If classified as Minimal Risk, you must submit a current updated evaluation that is no more than six months old at the time of the hearing. You must complete DUI Risk Education Course.
  • If classified as Moderate Risk, you must submit a current updated evaluation that is no more than six months old at the time of the hearing. You must complete DUI Risk Education Course and complete Early Intervention Program.
  • If classified as Significant Risk, you must submit a current updated evaluation that is no more than six months old at the time of the hearing. You must complete DUI Risk Education Course and must have documented completion of substance abuse treatment. Substance abuse treatment must include the following:
    • Individualized Treatment Plan
    • Discharge Summary
    • Aftercare/Continuing Care Plan
    • Continuing Status Report
  • If classified as High Risk, you must consult with DUI Evaluator to assess if and when are eligible for license reinstatement and what state mandates require for high risk offenders.
