DUI Services / Risk Education

What to bring to the Evaluation

  • Current photo ID or equivalent (not expired)
  • Traffic ticket showing the DUI charge
  • Blood Alcohol Level (BAC) print out from the testing machine, or documented refusal to be tested for BAC
  • Driver’s record from the Secretary of State—this must be the record/abstract “for court purposes”, and not the “public record” as provided to insurance companies.

Please note that after the DUI arrest, you may choose any service providers- see resource booklet for other provider listing in the area.

  • Categories for Education and Treatment required (see cost page for fees that are due at the first session)
  • Minimal Risk Risk Education only is required Minimal 10 hours required
  • Moderate Risk Risk Education and Substance Abuse Treatment is required 10 hours Risk education and 12 hours Substance Abuse Treatment
  • Significant Risk Risk Education and Substance Abuse Treatment is required 10 hours Risk Education and 20 hours Substance Abuse Treatment
  • High Risk Only Substance Abuse Treatment is required 75 hours Substance Abuse Treatment is required
